The Pitt: How HBO’s Hit Medical Drama Rescued the Genre

Like many US hospitals, Pittsburgh Trauma Medical Center (PTMC) is a place where time seems to dissolve. Rain or shine, any hour of the day, the environment is perpetually lit by harsh fluorescent lights. Wait times stretch for hours, and the lobby is plastered with warnings (“aggressive behavior will NOT be tolerated”), whereas televisions endlessly loop clips of shows like Deadliest Catch. As one observer place it, purgatory looks a lot like an American hospital… or, in this case, a meticulously constructed soundstage in Burbank, California.

The 52-bed ER set, currently bustling with infant actors preparing for a second-season scene, is the heart of The Pitt, the HBO Max medical drama that has unexpectedly revitalized the genre. The indicate has swept awards in the US and is finally making its debut in the UK. A flyer for a softball game between the cast and crew of Seattle Grace hints at the camaraderie on set.

Developed by the team behind the 1990s hit ER, The Pitt follows a diverse group of hospital workers – doctors, nurses, social workers, security personnel, and administrative staff – navigating the challenges of a financially strained emergency room in Pittsburgh. They confront everything from gunshot wounds and burnout to fentanyl overdoses and the often-dreaded task of paperwork, all while grappling with the emotional toll of their work.

The show, which premiered in the US in January 2025, achieved a rare feat: it became a word-of-mouth success due to its dramatic hyperrealism. Viewers were immersed in the unvarnished chaos of a US hospital, hour by hour. The series’ approach – prioritizing a single set and a relatively unknown cast to keep costs down – proved to be a double win, delivering an economically viable show in a contracting Hollywood landscape while simultaneously celebrating the dedication of healthcare workers. As series lead Noah Wyle described it, the show breaks through the cynicism of the sophisticated viewer. He spoke while holding a travel mug of black coffee and wearing a stethoscope, embodying the weariness and resilience of his character, Dr. Michael “Robby” Robinavitch.

A visit to the PTMC set reveals an astonishing level of detail. Every drawer is fully stocked, walkways are realistically cluttered, and every medication bottle is meticulously labeled (though filled with saline instead of actual drugs). Everything we say, everything we touch, is done as authentically as possible, explained Patrick Ball, who plays the chief resident Frank Langdon, highlighting the set’s dedication to realism, including a functioning scrubs-cleaning machine.

The blurring of reality and fiction has become a defining characteristic of The Pitt, transforming it into a form of addictive healthcare agitprop. The set, modeled after the real Allegheny General Hospital in Pittsburgh, is designed to create an optical illusion of endless hallways with no dead ends. The show’s unique concept – unfolding each season over a single ER shift, with each episode representing an hour in real time – has captivated millions of viewers.

Katherine LaNasa, who plays charge nurse Dana Evans and earned an Emmy for her performance, stays in character even between takes, jogging on the spot to maintain a breathless quality. It’s an exercise in respect for the profession, to try to do it accurately, she said. Extras, playing patients, scroll through their phones while awaiting their cues. The cast and crew are nearly indistinguishable, all wearing scrubs – a necessity for filming in a reflective environment and a deliberate effort to foster a sense of esprit de corps, as Wyle described it, a feeling that we’re all in this together to tell this story and we’re going to commit to being here for the next nine months.

This sense of camaraderie and decency in the face of systemic challenges is central to the show’s appeal. The workers at PTMC may argue, flirt, and struggle with their own flaws, but they are united by a commitment to caring for others. As Wyle put it, they strive to keep the light of humanity and decency and caring alive in what seem to be very dark times.

The show’s success has been remarkable. Viewers have embraced its rollercoaster rhythm and its unflinching portrayal of American social issues. Medical professionals have praised its accuracy and authenticity. It even won the Emmy for best drama, beating out more expensive and ambitious series like Severance, Andor, and The White Lotus.

Now, both seasons of The Pitt are available in the UK, as part of the launch of the streaming service HBO Max – a country perhaps unfamiliar with the complexities of the US healthcare system, but certainly no stranger to an overstretched health service. These are really confusing times, Wyle observed. And when you can tune in to something that feels vetted and representative of some semblance of reality, it’s like: ‘Oh, there’s a North Star.’ You can start to orient in a time where it’s very difficult to uncover your orientation.

The show’s creators emphasize that their goal is not to be polemical, but rather to be representational. They aim to depict the realities of an emergency room in Pittsburgh, without taking sides in political debates. They want to show the choices patients are forced to make, the impact of anti-science skepticism, and the challenges faced by healthcare providers, without offering easy answers or simplistic solutions.

The set itself is a testament to this commitment to realism. The blood mats – plasticized rugs designed to mimic puddles of blood – are used to minimize mess during mass casualty filming. The show’s inspiration stems from real-world experiences and expertise, with the writers’ room including two emergency room physicians and four on-set doctors, as well as eight to ten full-time nurses.

The show’s origins can be traced back to the COVID-19 pandemic, which inspired creator R. Scott Gemmill, John Wells, and Wyle to revisit the idea of an ER reboot. During the pandemic, they heard from healthcare professionals who felt unheard and misrepresented. There wasn’t a show that was realistic, Gemmill explained.

The journey to The Pitt wasn’t without its legal hurdles. The show is currently embroiled in a copyright lawsuit filed by the estate of Michael Crichton, creator of the original ER, alleging that The Pitt is essentially a reboot. Warner Bros. Has vehemently denied these claims, asserting that The Pitt is not a derivative work of ER.

Despite the legal challenges, the creators of The Pitt remain focused on telling authentic stories about the realities of healthcare. They hope to shed light on the ongoing challenges facing the US healthcare system, including the impact of the Affordable Care Act and the lingering trauma of the COVID-19 pandemic.

Season one of The Pitt is on HBO Max from March 26, with season two episodes available weekly.
