Pharmacists in France now have until April 30th, 2026, to pre-order flu vaccines for the upcoming vaccination campaign. This extension, announced by Sanofi, the manufacturer of Efluelda and Vaxigrip, comes after some initial hesitancy in pre-orders due to uncertainties surrounding vaccine pricing and distribution. The move aims to provide pharmacies with more time to finalize their orders following recent clarifications from health authorities.

Easing Uncertainty for Pharmacies

Late February saw the French Directorate-General of Health (DGS) express concern over the slow pace of flu vaccine pre-orders by pharmacies. Le Quotidien du Pharmacien reports that these delays stemmed from questions about potential changes impacting order volumes. Specifically, pharmacies were awaiting clarity on two key issues: a proposed unified margin for all flu vaccines and the distribution model involving other healthcare professionals.

On March 24th, the Ministry of Health addressed these concerns. Crucially, the plan to implement a single margin for all flu vaccines – including standard options like Vaxigrip and Influvac, and enhanced vaccines like Fluad and Efluelda – has been abandoned. While other healthcare providers authorized to administer flu vaccines will be permitted to stock doses directly, their supply will be sourced exclusively through pharmacies. This ensures pharmacies remain central to the vaccine distribution network.

Sanofi Extends Pre-Order Deadline

With these uncertainties resolved, Sanofi has extended its pre-order campaign deadline from March 31st to April 30th, 2026. This provides pharmacies with additional time to adjust their order quantities based on the clarified guidelines. Other vaccine manufacturers, including CSL Seqirus (Flucelvax) and Viatris (Influvac), are expected to follow suit and extend their respective pre-order deadlines, pending official confirmation from health authorities.

Efluelda and the High-Dose Vaccine Landscape

The situation highlights the importance of high-dose influenza vaccines, particularly for older adults. Sanofi’s Efluelda is a trivalent influenza vaccine designed for individuals aged 60 years and older. It delivers 60 micrograms of hemagglutinin (HA) per strain, a significantly higher dose than the 15 micrograms found in standard-dose vaccines. Sanofi emphasizes that Efluelda offers better protection against both influenza and flu-related hospitalizations compared to standard-dose options. Recent research, including a study cited by Sanofi, suggests improved efficacy against lab-confirmed influenza in older adults.

However, it’s important to note that Efluelda is facing market changes. According to a document from the French Ministry of Labor, Health and Solidarity (DGS-URGENT), the vaccine will be withdrawn from the market in the coming weeks. Sanofi has committed to replacing all pre-orders of Efluelda with alternative influenza vaccines, ensuring continued supply for the upcoming vaccination season. This withdrawal doesn’t signal a safety concern, but rather a strategic shift in Sanofi’s product portfolio.

Superior Protection in Older Adults: Evidence and Context

The rationale behind high-dose vaccines like Efluelda lies in the age-related decline in immune response. As individuals age, their immune systems turn into less effective at responding to influenza vaccines. A higher antigen dose aims to overcome this diminished response, providing better protection. A study highlighted by Sanofi demonstrates that high-dose influenza vaccines offer superior protection against hospitalizations due to pneumonia or influenza, as well as hospitalizations related to cardio-respiratory events, in older adults. Sanofi’s press release details these findings.

It’s crucial to understand that vaccine efficacy is not absolute. While high-dose vaccines demonstrate improved efficacy compared to standard-dose vaccines, they do not guarantee complete protection against influenza. Factors such as the match between the vaccine strain and circulating strains, individual health status, and adherence to public health measures all influence vaccine effectiveness. Studies evaluating vaccine efficacy often have limitations, including potential biases and the challenges of accurately measuring influenza infection rates.

The extension of the pre-order deadline and the ongoing availability of alternative influenza vaccines are positive developments for public health in France.
