Spain’s IMPaCT Initiative: Advancing Precision Medicine Through Data and Collaboration

Spain is making significant strides in precision medicine with the continued growth of the Infrastructure of Precision Medicine associated with Science and Technology (IMPaCT). Launched five years ago, IMPaCT is now a cornerstone of the nation’s strategy to translate biomedical research into clinical practice. The initiative focuses on predictive medicine, data science, and genomics, aiming to deliver more personalized, equitable, and evidence-based healthcare. This progress is being driven by the Instituto de Salud Carlos III (ISCIII), with support from the Ministry of Science, Innovation and Universities, and the Ministry of Health.

At the heart of IMPaCT is a commitment to leveraging data to improve health outcomes. A key component is the IMPaCT Cohort, an ambitious study aiming to collect data from 200,000 individuals over a 20-year period. This large-scale data collection effort is intended to identify risk factors for major diseases, inform prevention strategies, and enhance the overall health of the Spanish population.

Building a National Cohort for Predictive Health

The IMPaCT Cohort is already making headway, with over 26,000 participants recruited from 51 primary healthcare centers across Spain. Recruitment is designed to be representative of the country’s diverse population, both socially, and demographically. Participants undergo clinical tests, provide samples, and complete questionnaires about their lifestyles. This data – genomic, clinical, and lifestyle-related – is anonymized and used for research purposes. The goal is to anticipate health risks, develop personalized prevention plans, and improve health outcomes for all citizens. If contacted by a local health center to participate, individuals are encouraged to contribute to this important scientific endeavor.

The Cohort’s approach is similar to other large-scale biobank initiatives globally, such as the UK Biobank (UK Biobank), which has provided valuable insights into the genetic and environmental factors influencing health. However, IMPaCT is specifically tailored to the Spanish population and healthcare system.

Data Science as a Catalyst for Innovation

Beyond the Cohort, IMPaCT is investing heavily in data science infrastructure. The initiative aims to create a common, interoperable system for collecting and analyzing biomedical data, drawing on existing resources within the Spanish science and technology ecosystem. This will enable researchers to address complex questions and generate new insights from diverse data sources. A key focus is ensuring that researchers have access to population-level health data based on individual patient information.

The program is developing recommendations and best practices for collecting electronic health record information, ensuring interoperability across different systems. It’s also creating a virtual research environment for analyzing genomic, clinical, and medical imaging data, and identifying or developing software tools for federated data analysis.

Advancing Genomic Medicine for Rare Diseases and Cancer

IMPaCT’s genomics section is focused on improving diagnosis and treatment for rare diseases, cancer, and other genetic conditions. The program supports whole genome sequencing for individuals with rare diseases or familial cancer where a genetic cause hasn’t been identified through standard testing. By employing advanced genomic analysis techniques, IMPaCT aims to provide more accurate diagnoses and pave the way for new therapies.

To date, IMPaCT Genómica has sequenced the genomes of over 2,000 patients with undiagnosed rare diseases and those suspected of having hereditary cancer. This work involves collaboration with over 300 professionals and a network of 110 hospitals throughout Spain.

Citizen Engagement: A Cornerstone of Success

Recognizing the importance of public participation, ISCIII has integrated citizen involvement into IMPaCT from its inception. This includes engaging with patient organizations like the Plataforma de Organizaciones de Pacientes (Plataforma de Organizaciones de Pacientes), the Federación Española de Enfermedades Raras (Feder) (Federación Española de Enfermedades Raras), and the Asociación Española Contra el Cáncer (AECC). The initiative emphasizes open science – accessible, efficient, democratic, transparent, and dynamic – with participation from both the scientific community and the public.

The positive response to the IMPaCT Cohort demonstrates the public’s willingness to contribute. Approximately 50% of individuals contacted by their healthcare center to participate in the Cohort agree to do so, a rate that surpasses similar international initiatives.

Coordination and Collaboration: A National and International Network

IMPaCT’s success relies on effective coordination across its various nodes and programs. ISCIII oversees the initiative, conducting regular follow-up meetings, scientific conferences, and information dissemination events. Internationally, IMPaCT collaborates with transnational initiatives like Genome of Europe, contributing genomic data from the Cohort to advance global understanding of genetic variation and disease.

Data management is a critical aspect of IMPaCT. The ISCIII biobank stores samples, and efforts are underway to centralize genomic, imaging, and clinical data for broader research leverage. This includes developing guidelines for genomic analysis and data management, as well as models for diagnostic reporting and clinical recommendations, all while safeguarding individual privacy.

Looking Ahead: Continued Growth and Impact

IMPaCT is poised for continued growth, with plans to develop tools for visualizing Cohort data, expand the network of genotyping centers, and create a data lake for research projects. The three IMPaCT programs will continue to generate knowledge and evidence to support personalized medicine in Spain. The initiative’s long-term vision is to establish a robust and sustainable infrastructure for precision medicine, ultimately improving the health and well-being of the Spanish population. The continued development of IMPaCT is vital for deepening our understanding of disease and providing the best possible care.
