New Drug Approved to Reduce Breast Cancer Recurrence Risk in Italy

The approval of a new drug by the Italian Medicines Agency (AIFA) offers renewed hope for women and men diagnosed with early-stage breast cancer. The medication, administered alongside standard hormone therapy for three years, has demonstrated significant efficacy in reducing the risk of recurrence and distant metastasis – even decades after initial diagnosis.

More than 55,000 Italians are diagnosed with breast cancer annually, but advancements in early detection indicate nearly nine out of ten patients now achieve remission. However, the possibility of cancer returning remains a significant concern. “Recurrence can occur even when the tumor is discovered in the early stages; it’s one of the biggest fears for every patient,” explains Michelino De Laurentiis, Director of the Department of Breast and Thoracic Oncology at the National Tumor Institute IRCCS Fondazione Pascale in Naples. Approximately one in five patients experiences a return of the disease within five years of initial treatment, often presenting as metastatic cancer.

Understanding the Risk and the New Treatment

The new drug, ribociclib, is a targeted therapy that works in conjunction with hormone therapy. The approval is based on data from the NATALEE study, which involved over 5,000 individuals with invasive ductal adenocarcinoma, hormone receptor-positive (HR+), and HER2-negative breast cancer in stages II and III, identified as having a risk of recurrence with positive lymph nodes. Standard treatment for these patients typically involves hormone therapy for 5-10 years, potentially preceded by chemotherapy. Participants in the NATALEE trial received three years of standard hormone therapy plus ribociclib.

The results, updated five years after the trial’s conclusion and presented at the European Society for Medical Oncology (ESMO) congress last October, are promising. Over 85% of patients treated with ribociclib remained free of recurrence after five years, compared to 81% of those receiving hormone therapy alone. Crucially, the risk of recurrence was reduced by 28%, and the risk of developing distant metastasis – the primary cause of mortality in breast cancer – decreased by approximately 30%.

How Ribociclib Works and its Side Effects

Ribociclib belongs to a class of drugs called cyclin-dependent kinase inhibitors. Adding this targeted therapy to standard hormone therapy significantly improves survival rates and reduces the risk of recurrence and metastasis in patients with early-stage, hormone-sensitive, HER2-negative breast cancer – the most common subtype, accounting for around 70% of the 55,000 new cases diagnosed annually in Italy. Importantly, ribociclib is generally well-tolerated, with most side effects being asymptomatic, meaning they don’t cause noticeable symptoms. The primary side effect is neutropenia, but the drug appears to have a minimal impact on patients’ quality of life.

Long-Term Recurrence and the Importance of Vigilance

While hormone-sensitive breast cancers typically have a good short-term prognosis, recurrence can occur many years after initial diagnosis, even after two decades. “The event that most impacts clinical outcomes in surgically treated breast cancer is the appearance of distant recurrence,” explains Giuseppe Curigliano, President-Elect of ESMO and Director of New Drug Development for Innovative Therapies at the European Institute of Oncology in Milan. “This new treatment is valuable given that preventing a possible return of the tumor is crucial for improving the chances of cure for a growing number of patients and avoiding the clinical, social, and economic consequences associated with disease progression to metastatic forms.”

Statistics indicate that one in eight women will develop breast cancer during their lifetime. Approximately 10% of diagnoses occur in women under 40, and the majority of cases (around 70%) are hormone-sensitive. Recent studies, particularly in the United States, suggest an increase in cancer cases among younger individuals, those under 50.

Recognizing Potential Warning Signs

Early diagnosis significantly improves the chances of successful treatment, with survival rates approaching 90%. It’s vital not to ignore potential warning signs. The most common symptom is a painless lump with irregular contours. Other frequent signs include swelling in part or all of the breast, changes in skin texture (such as an orange peel appearance), alterations in breast shape (including indentations), changes to the nipple (turning inward or outward), fluid or blood discharge from the nipple, and swelling of lymph nodes in the armpit, collarbone, or neck. Consulting a doctor for evaluation is essential if any of these symptoms are present.

The introduction of ribociclib represents a significant step forward in breast cancer treatment, offering a new layer of protection against recurrence and improving long-term outcomes for patients. Continued surveillance and adherence to recommended screening guidelines remain crucial for early detection and effective management of this disease.
