Improved patient safety is becoming a reality in Region Östergötland thanks to the implementation of new antibiotic concentration measurements. This advancement allows healthcare professionals to more accurately tailor antibiotic dosages to individual patients, maximizing effectiveness even as minimizing potential side effects and the development of antibiotic resistance. The initiative reflects a growing emphasis on precision medicine and responsible antibiotic stewardship within the region’s healthcare system.

Understanding Antibiotic Concentration Measurements

Antibiotic concentration measurements involve analyzing the levels of antibiotics in a patient’s bloodstream. Traditionally, antibiotic dosages have been determined based on factors like weight, kidney function, and the type of infection. However, these estimations don’t always account for individual variations in how the body absorbs, distributes, metabolizes, and eliminates drugs – a process known as pharmacokinetics. By directly measuring antibiotic concentrations, clinicians can ensure that patients receive optimal doses, avoiding both under-treatment (where the drug isn’t effective) and over-treatment (which can lead to toxicity and resistance).

The Strama Östergötland initiative, part of the national Stramanätverket, is central to this effort. Strama Östergötland collaborates with healthcare facilities to preserve the effectiveness of antibiotics and reduce the risk of resistance development. This includes developing action proposals to reduce healthcare-associated infections and slow the spread of resistance.

Who Benefits from This Change?

The primary beneficiaries are patients receiving antibiotic treatment, particularly those with severe or complex infections. Accurate dosing is especially crucial for individuals with compromised immune systems, kidney or liver problems, or those undergoing intensive care. The new measurements are being implemented across hospitals in Region Östergötland, impacting both inpatient and outpatient care. The initiative similarly supports the work of the Strama-ansvarig läkare (SAL), or Strama-responsible physician, who is jointly responsible with the head of operations for the local antibiotic work within clinics and healthcare centers.

The Role of the Strama App and Other Resources

To support healthcare professionals in implementing these changes, Region Östergötland provides several resources. The Strama app offers treatment recommendations for antibiotics, accessible on both mobile phones and web pages. Healthcare providers can search for “Strama nationell” in app stores to download the application. The Regnbågsbroschyren (Rainbow Brochure) provides concise summaries of treatment recommendations for common infections in outpatient settings. The “Gröna kortet” (Green Card) and “Röda kortet” (Red Card) offer guidance on empirical antibiotic treatment for common and life-threatening infections, respectively.

Monitoring and Data Collection

Region Östergötland is actively monitoring the impact of these new measurements through data collection and analysis. For primary care providers, statistics on antibiotic prescriptions linked to diagnoses are available in Rebus vård, under the “patientsäkerhet” (patient safety) category. Hospital staff can access open statistics in the Infektionsverktyget (Infection Tool), with access to more detailed data available through their local behörighets administratör (LBA) – the local authorization administrator. Further data on antibiotic sales and resistance can be found through the Swedish Public Health Agency.

Antibiotic Resistance: A Broader Context

The focus on optimizing antibiotic use is driven by the growing threat of antibiotic resistance. Resistant bacteria continue to increase, posing a significant risk to public health both in Sweden and internationally. The link between antibiotic use and resistance development is well-established, but reducing antibiotic prescribing alone isn’t enough. A comprehensive approach, like the 10-point program for reduced antibiotic resistance promoted by Strama, is needed to ensure effective infection treatment in the future. This program emphasizes responsible antibiotic use, infection prevention, and reduced transmission.

The implementation of antibiotic concentration measurements is an ongoing process. Region Östergötland will continue to evaluate the effectiveness of this approach, refine protocols based on data analysis, and provide ongoing training and support to healthcare professionals.
