Rare Case: Man in India Discovered to Have a Third Kidney During UTI Treatment

Rare Case: Man in India Discovered to Have a Third Kidney During UTI Treatment

March 21, 2026 Nkechi Okonkwo- Health Editor Health

A 31-year-classic man in India recently received a surprising diagnosis whereas seeking treatment for a severe urinary tract infection (UTI). Medical imaging revealed the presence of a third kidney, a rare condition known as a supernumerary kidney, uniquely fused with his right kidney and resembling a horseshoe shape. The case highlights the complexities of anatomical variation and the potential for unexpected findings during routine medical investigations.

The patient initially presented with a five-day history of intense lower right back pain, a fever reaching 39 degrees Celsius, and a burning sensation during urination. Laboratory tests confirmed a Klebsiella pneumoniae bacterial infection, a common cause of UTIs, and indicated active infection through cloudy urine analysis. While his vital signs were generally stable, further investigation with ultrasound and CT scans unveiled the unusual anatomical feature.

Understanding Supernumerary Kidneys

A supernumerary kidney, or kidney triplicate, is a congenital anomaly believed to occur during fetal development due to abnormal cell division in the umbilical cord. As reported by detikHealth, this division can lead to the formation of an additional kidney. While often asymptomatic and undetected, these extra kidneys can sometimes predispose individuals to kidney infections. The condition is relatively rare, and its prevalence in the general population is not well-established.

In this particular case, the supernumerary kidney was fused with the patient’s right kidney, creating a distinctive horseshoe-like structure. This anatomical variation, while unusual, doesn’t automatically imply impaired kidney function. However, it can alter urinary flow and potentially increase the risk of complications like infections or blockages.

Beyond the Third Kidney: Additional Complications

The diagnostic process revealed more than just the presence of a third kidney. Both the left and right kidneys exhibited swelling and the accumulation of mineral crystals, known as calculi or kidney stones. The most significant concern was a blockage in the left ureter – the tube connecting the kidney to the bladder – caused by these crystal deposits. This obstruction could impede urine flow and potentially lead to kidney damage.

To address the ureteral blockage, doctors inserted a stent, a thin plastic tube, to facilitate urine drainage. The patient also received intravenous antibiotics – piperacillin and tazobactam, along with levofloxacin administered every other day – to combat the UTI.

Treatment and Recovery

Following 48 hours of intensive treatment and hydration, the patient’s condition improved significantly. He was discharged with a follow-up appointment scheduled for one month to remove the stent and continue management of the kidney stones. Media Indonesia reports that this approach aims to restore normal urinary function and prevent further complications.

What Does This Mean for Kidney Health?

This case underscores the importance of thorough investigation when patients present with seemingly straightforward conditions like UTIs. The discovery of a supernumerary kidney, while rare, highlights the anatomical variability that can exist within the human body. It also emphasizes the potential for underlying structural abnormalities to contribute to urinary tract issues.

Kidney stones, a common co-occurring condition in this case, form when certain minerals become concentrated in the urine. Factors contributing to kidney stone formation include dehydration, diet, and underlying medical conditions. Symptoms can range from mild discomfort to severe pain, and complications can arise if stones block the urinary tract. OB News details the patient’s experience with this aspect of his condition.

The Diagnostic Process: USG and CT Scans

The accurate diagnosis relied heavily on the use of imaging techniques – ultrasound (USG) and computed tomography (CT) scans. Ultrasound uses sound waves to create images of internal organs, while CT scans utilize X-rays to generate detailed cross-sectional images. Both modalities played a crucial role in identifying the supernumerary kidney and assessing the extent of the ureteral blockage. It’s important to note that CT scans involve exposure to ionizing radiation, and their use is typically reserved for situations where the benefits outweigh the risks.

Looking Ahead: Continued Monitoring and Management

The patient’s case will require ongoing monitoring to ensure the continued function of all three kidneys and to manage the kidney stones effectively. Regular follow-up appointments will be essential to assess kidney function, monitor for any signs of infection or blockage, and adjust treatment as needed. The removal of the stent is a crucial step, and long-term management of kidney stones may involve dietary modifications, increased fluid intake, or further medical interventions if necessary.

This case serves as a reminder that even common ailments can sometimes reveal unexpected anatomical variations. A comprehensive diagnostic approach, coupled with appropriate treatment and follow-up care, is essential for ensuring optimal kidney health and overall well-being.
