Wake-Promoting Drug Boosts Alertness in Early Morning Shift Workers – Clinical Trial

Millions of people across the country begin their workdays before sunrise, and for many, the struggle isn’t a lack of motivation, but a biological mismatch. A new clinical trial offers a potential solution for those grappling with excessive sleepiness due to early morning shifts, suggesting that the wake-promoting drug solriamfetol (Sunosi) can improve alertness without disrupting later sleep. This research, published in NEJM Evidence, specifically addresses a gap in understanding and treatment for those whose schedules demand they rise while their bodies are still primed for rest.

The Challenge of Early Morning Work

Approximately one in four workers operate on schedules outside the traditional 9-to-5, and many of these individuals – often not identifying as traditional “shift workers” – face the challenges of shift work disorder. This condition is characterized by disrupted sleep and excessive daytime sleepiness, impacting concentration, job performance, and safety. Researchers at Mass General Brigham emphasize that the issue is particularly acute for those starting work between 3 a.m. And 7 a.m., when the brain is naturally programmed for sleep. Mass General Brigham researchers found that solriamfetol improved alertness in early morning shift workers with shift work disorder.

“People who start work between 3 a.m. And 7 a.m. Are waking up at a time when the brain is biologically programmed to sleep. That makes staying alert extraordinarily difficult, even when they are highly motivated,” explained first author Kirsi-Marja Zitting, PhD, an investigator with the Division of Sleep and Circadian Medicine in the Mass General Brigham Department of Medicine. This “double burden” of sleepiness during work and difficulty sleeping when the opportunity arises can significantly impact quality of life.

How Solriamfetol Works and Why It’s Different

Shift work disorder can elevate the risk of impaired cognitive function, reduced productivity, motor vehicle accidents, and workplace injuries. While wake-promoting medications like modafinil are sometimes prescribed, they’ve primarily been studied in overnight shift workers and can potentially interfere with subsequent sleep. Solriamfetol, already approved for treating excessive sleepiness associated with obstructive sleep apnea and narcolepsy, was considered a promising alternative as of its potential to promote wakefulness for several hours without significantly hindering later sleep. SciTechDaily reports that the drug targets neurotransmitters involved in wakefulness.

The Clinical Trial Design and Results

The clinical trial involved 78 early morning shift workers diagnosed with shift work disorder. Participants were randomly assigned to receive either solriamfetol or a placebo daily for four weeks. Researchers assessed participants’ ability to stay awake in a quiet, dark environment during simulated work hours and collected data on their daily functioning through physician evaluations and self-reporting.

After the four-week period, the group receiving solriamfetol reported significantly reduced sleepiness and demonstrated a longer duration of wakefulness during simulated work hours. Both participants and their physicians observed improvements in overall functioning, work productivity, and daily activities. “The improvement we saw is clinically meaningful,” stated senior author Charles A. Czeisler, PhD, MD, chief and senior physician of the Division of Sleep and Circadian Medicine in the Mass General Brigham Department of Medicine. “These workers were able to stay awake and alert throughout a full eight-hour shift, which has real implications for performance, safety, and quality of life.”

Understanding the Study’s Limitations and Future Research

While the findings are encouraging, researchers acknowledge certain limitations. The study focused on otherwise healthy adults, and the long-term effects of solriamfetol remain unknown. The trial specifically examined early morning shift workers. the effects on those working overnight shifts require further investigation. Medical Xpress notes that the study addressed a significant gap in research, as previous trials had not specifically targeted early morning shift workers.

The research team is currently recruiting participants for a follow-up trial to evaluate solriamfetol’s effectiveness in overnight shift workers. This additional research could potentially pave the way for the drug’s approval as a treatment for shift work disorder across various work schedules.

What This Means for Shift Workers and Healthcare Providers

This study offers a glimmer of hope for the millions of individuals whose work schedules disrupt their natural sleep-wake cycles. While solriamfetol is not a cure-all, it represents a potential tool for managing the debilitating effects of shift work disorder. It’s important to remember that this medication should only be considered under the guidance of a qualified healthcare professional.
