When to Suspect TB in Children: Symptoms & Treatment

But, there are instances where greater vigilance is needed, particularly if the cough doesn’t improve. Determining whether a child’s cough is indicative of TBC requires careful observation, according to Dr. Lucy Endang Savitri, a Pediatric Specialist at Jogja International Hospital (JIH) Solo. “Assessing a child’s cough isn’t solely based on the cough itself. There are several factors parents should monitor at home,” Dr. Lucy explained in a recent interview with HaiBunda.

A key indicator is the duration of the cough. If a cough persists for more than two weeks and doesn’t respond to standard, non-TBC treatments, further investigation is warranted. “If a cough lasts longer than two weeks and doesn’t improve with non-TBC medications, and is accompanied by a fever lasting over two weeks that also doesn’t respond to non-TBC treatment, we should consider TBC as a possibility,” she clarified.

The health status of those around the child is also important. If a family member who frequently interacts with the child is diagnosed with TBC, it increases the child’s risk. Swollen lymph nodes, measuring over 1 cm in diameter around the neck, can be a sign.

Monitoring a child’s growth is another crucial aspect. If a child’s weight remains stagnant despite adequate nutrition and care, it could be a cause for concern. “If a child’s weight doesn’t increase despite efforts to improve their nutrition and address other potential causes, we should suspect TBC and seek medical evaluation,” Dr. Lucy added.

Is TBC in Children Curable?

The Indonesian Pediatric Association (IDAI) confirms that TBC in children is indeed curable. However, treatment is a lengthy process, typically lasting six to twelve months, depending on the severity of the infection. TBC medication is most effective when taken on an empty stomach to maximize absorption. A one-hour interval before or after meals is recommended. Treatment usually involves a combination of three to four different drugs, available as separate pills or as fixed-dose combinations, often formulated as chewable tablets or solutions for easier administration to children.

Beyond medication, a supportive environment is crucial for recovery. This includes a nutritious diet, a clean and well-ventilated home, and ensuring the child has received the Bacillus Calmette-Guérin (BCG) vaccine as an infant.

Rising TBC Cases in Indonesia and Jakarta

The concerns about childhood TBC arrive amidst a broader increase in TBC cases across Indonesia. According to the Ministry of Health, Indonesia recorded 1.06 million cases of TBC in 2024, the highest number ever reported. This surge is partly attributed to improved detection and reporting systems, but a significant factor is the impact of the COVID-19 pandemic, which disrupted TBC care for adults, potentially leading to increased transmission to children.

In Jakarta Timur (East Jakarta), a recent outbreak has raised particular alarm. Reports from May 2025 indicate that 2,645 residents were infected with TBC within three months, including 324 children. This highlights the urgent need for proactive public health measures and increased awareness.

Understanding TBC Transmission and Prevention

TBC is caused by the bacterium Mycobacterium tuberculosis, which can infect any part of the body, not just the lungs. Transmission occurs when airborne droplets containing the bacteria are inhaled. The BCG vaccine, administered to infants, provides some protection against severe forms of TBC, but its effectiveness varies.

The 2023 guidelines for managing TBC in children and adolescents in Indonesia emphasize early detection, prompt treatment, and contact tracing to prevent further spread.
