The NIVIPIT trial, a phase 1b randomized multicenter study involving 61 patients, sought to address this challenge. Researchers discovered that by administering ipilimumab (anti-CTLA4) directly into the tumor, while continuing nivolumab (anti-PD1) intravenously, they could significantly reduce the incidence of grade 3 or 4 treatment-related adverse events – bringing it down to 22.6% compared to 57.1% with intravenous ipilimumab alone. Researchers found that baseline tumor immune profiling revealed that protumoural activated regulatory T (Treg) cells and M2 macrophages predicted durable clinical benefit, regardless of how the anti-CTLA4 was administered. Interestingly, a decrease in activated intratumoural Treg cells was only observed in patients who experienced lasting clinical improvement, and these patients as well exhibited high intratumoural Fcγ receptor (FcγR) expression.

The Immune Landscape and Predictive Biomarkers

The study’s deeper dive into the tumor microenvironment is particularly noteworthy. The research team, utilizing transcriptomic deconvolution, immunohistochemistry (IHC), and flow cytometry, identified key immune markers associated with treatment response. Patients who experienced durable clinical benefit (DCB) showed significantly higher infiltration of T cells at baseline. While total CD8+ T cell infiltration didn’t differ significantly between groups, the DCB group had a higher frequency of CD8+PD1+ T cells. This suggests that a specific subset of exhausted or activated CD8+ T cells may serve as a predictive biomarker for immunotherapy response. This is a critical area of ongoing research, and the findings could eventually lead to personalized treatment strategies.

The implications extend beyond melanoma. The researchers suggest that intratumoral anti-CTLA4 strategies could be beneficial in oligometastatic and early-stage cancers. Understanding these prerequisites is crucial for identifying patients who are most likely to benefit from this treatment modality.
