Bronchiectasis: Clear Mucus & Improve Breathing – Techniques & Devices

Bronchiectasis: Clear Mucus & Improve Breathing – Techniques & Devices

March 3, 2026 Ananya Mittal - World Editor News

For those living with bronchiectasis, a chronic lung condition marked by widened and thickened airways, managing mucus buildup is central to treatment. Effective airway clearance can significantly lower the risk of infection, ease breathing, minimize hospital visits, reduce flare-ups, and potentially leisurely disease progression. Understanding the available techniques – from simple breathing exercises to mechanical devices and physiotherapy – is a crucial step in navigating life with this condition.

The Core Principle: Clearing the Airways

“Making sure you’re clearing the mucus that’s in the lung is what reduces the risk of recurrence of infections,” explains Amit “Bobby” Mahajan, MD, system chief of interventional pulmonology at Inova Health System and national medical spokesperson for the American Lung Association. This principle underpins all bronchiectasis treatment strategies. Bronchiectasis occurs when damage to the airways prevents them from effectively clearing mucus, leading to bacterial buildup and recurrent infections. Techniques focused on mobilizing and removing this mucus are paramount.

Active Cycle of Breathing Technique (ACBT): A Daily Ritual

The Active Cycle of Breathing Technique (ACBT) is a widely recommended, accessible method for clearing mucus. It’s a flexible routine that doesn’t require specialized equipment and can be adapted to individual needs. ACBT operates on a three-step cycle: breath control (slow, relaxed belly breathing to maintain open airways), deep breathing (slow, deep breaths with a brief pause to loosen mucus), and huffing (a forceful exhale through an open mouth, like fogging a mirror, to move mucus upwards).

Research indicates that ACBT can be as effective as many airway clearance devices when used consistently, leading to improved breathing and fewer flare-ups. Most individuals with bronchiectasis perform ACBT once or twice daily when stable, increasing frequency during exacerbations. A typical session lasts 10 to 30 minutes.

Mechanical Assistance: Airway Clearance Devices

When breathing exercises alone aren’t sufficient, mechanical devices can provide additional assistance in loosening and moving mucus. These devices are often used in conjunction with breathing techniques as part of a comprehensive daily routine.

Oscillating Positive Expiratory Pressure (OPEP) Devices

OPEP devices, often described as resembling a compact whistle, create both gentle resistance and vibrations when exhaled through. Dr. Mahajan explains that these vibrations help thin mucus and propel it upwards, effectively reducing symptoms and the frequency of flare-ups.

High-Frequency Chest Wall Oscillation (HFCWO) – “The Vest”

High-frequency chest wall oscillation (HFCWO), commonly known as “the vest,” is a wearable device that rapidly vibrates to gently compress and release the chest. This movement helps dislodge thick mucus from the smaller airways, moving it into larger spaces where it can be more easily expelled. HFCWO is particularly beneficial for individuals who find manual breathing exercises or handheld tools physically demanding. Consistent use has been shown to reduce flare-ups, decrease the need for antibiotics, and minimize hospitalizations related to bronchiectasis.

Positive Expiratory Pressure (PEP) Devices

PEP devices utilize a mask or mouthpiece to create gentle back pressure during exhalation. This pressure helps keep the airways open, allowing air to get behind the mucus and facilitate its expulsion. Studies suggest that PEP therapy is comparable in effectiveness to other clearance methods in maintaining lung function and improving quality of life, provided it’s used regularly.

Intrapulmonary Percussive Ventilation (IPV)

IPV employs a machine to deliver a fine mist, sometimes with medication, alongside gentle, rapid pulses of air to the lungs. These pulses create vibrations within the airways, aiding in mucus mobilization and expulsion.

The Role of Physiotherapy and Manual Techniques

The European Respiratory Society’s clinical practice guidelines emphasize the importance of involving a physiotherapist experienced in bronchiectasis as part of a comprehensive care plan. A physiotherapist can help initiate a personalized routine using intensive, hands-on techniques and provide guidance on the most effective airway clearance methods for individual needs.

Chest Physical Therapy (CPT)

CPT involves the therapist using cupped hands to rhythmically clap on the back and chest, helping to loosen mucus from the lungs. They can also advise on the use of electronic chest clappers or vests for home use.

Postural Drainage

Postural drainage involves positioning the body in specific ways, sometimes with the head below the waist, to utilize gravity to drain mucus from different lung regions towards the throat. A physiotherapist can assess lung congestion and recommend the most effective positions for each individual.

Understanding Bronchiectasis and Seeking Support

Bronchiectasis is a chronic condition, meaning it requires ongoing management. Early diagnosis and treatment, coupled with consistent airway clearance techniques, are vital for preventing further lung damage. The American Lung Association provides comprehensive information and resources for individuals living with bronchiectasis and their families. If you suspect you may have bronchiectasis, it’s crucial to consult with a qualified healthcare professional for proper diagnosis and a personalized treatment plan.

Further research into novel therapies and personalized treatment approaches continues, offering hope for improved outcomes and quality of life for those affected by this challenging condition.
