Dementia Care Improves with New Medicare Program – Bluestone Physician Services

The challenges facing families navigating dementia care are becoming increasingly visible, even here in Minnesota. A Stillwater-based care provider, Bluestone Physician Services, is among the first in the nation to participate in a novel federal program designed to address this growing crisis. This isn’t just a national story; it’s one that’s directly impacting families across the Twin Cities and beyond, as the number of individuals needing specialized care continues to rise.

The Rising Tide of Dementia and the Strain on Families

Jessie Waks, Senior Vice President and Head of Primary Care at Bluestone, has observed a significant trend over her 17 years with the company: a growing demand for senior living communities and, crucially, for dementia-specific care within those communities. While the increase in available resources seems positive on the surface, it underscores a deeper problem. Waks points out that even with more options, families often find themselves overwhelmed, taking on responsibilities they aren’t equipped to handle, even when their loved ones are in assisted living.

The core issue, as Waks explains, is a lack of comprehensive explanation and support. Too often, patients reach memory care without a clear understanding of their diagnosis, the progression of the disease, or what to expect. This gap in communication leaves families scrambling to navigate a complex healthcare system, often feeling lost and alone. The demands of dementia care – requiring more frequent and intensive attention than many traditional healthcare scenarios – further exacerbate the strain.

A National Crisis: Statistics and the GUIDE Program

The statistics paint a stark picture. Currently, 6.7 million Americans live with some form of dementia. However, projections indicate a dramatic increase, with nearly 14 million U.S. Dementia patients expected by 2060. This growth is largely attributed to lengthening lifespans, as dementia predominantly affects older populations. Recognizing the urgency of the situation, the Centers for Medicare & Medicaid Services (CMS) launched the Guiding an Improved Dementia Experience (GUIDE) program in July 2024.

Bluestone Physician Services was selected to participate in the GUIDE program in August 2024, beginning service delivery in July 2025. The program’s innovative approach centers around assigning “care navigators” to work alongside physicians, providing dedicated support to eligible patients. Crucially, GUIDE also offers up to $2,500 in respite benefits, providing temporary relief for unpaid caregivers – often family members – who shoulder a significant portion of the care burden.

How GUIDE is Changing Care at Bluestone

The impact of GUIDE at Bluestone is already being felt. Waks explains that the program has allowed them to expand their care team by 20 members, including the vital care navigators. This additional staffing allows Bluestone’s primary care providers – physicians, nurse practitioners, and physician assistants – to focus on the medical and clinical aspects of care, while the navigators provide more personalized support to patients and their families.

This support can take many forms, including in-home visits to assist patients experiencing anxiety or restlessness. Waks emphasizes the importance of maintaining patients’ connection to their homes and communities, stating, “Bringing a patient with dementia out of their home can cause increased confusion and chaos… We know that [home] is the best place for chronically ill patients… Our goal is to keep them home as much as possible.” The program is also providing Bluestone with a new revenue stream, allowing them to address gaps in care that previously went unmet.

Navigating Potential Challenges: Medicare Funding and the Future of Dementia Care

Despite the positive momentum, potential headwinds loom. The recently passed One Huge Beautiful Bill Act, signed into law in July 2025, is projected to slash Medicare funding by $536 billion over the next decade. While the full impact on the GUIDE program remains uncertain, Waks acknowledges the critical importance of continued support, particularly for patients who often represent a significant cost to the healthcare system. Bluestone currently serves approximately 18,000 patients in primary care, spread across 600 assisted living and group homes in Minnesota, with a growing presence in Wisconsin and Florida. The company employs around 600 individuals.
