Drugs Double Stroke Risk, Especially in Young People: Major Study Reveals Links
Recreational drug use is linked to a significantly increased risk of stroke, even in younger adults, according to a major review of existing research. The analysis, encompassing data from over 100 million individuals, reveals a more than doubling of stroke risk for users of amphetamines and a near doubling for cocaine users. While the findings highlight the dangers of illicit drug use, they also underscore a gap in public awareness regarding these risks, particularly among young people.
Amphetamines and Stroke: A Tripling of Risk for Younger Users
The study, published in the International Journal of Stroke, found that amphetamine use was associated with a 122% higher risk of stroke overall. Still, the risk was particularly pronounced in individuals under the age of 55, where it nearly tripled. Cocaine use showed a 96% increase in stroke risk across all ages, and a similar 97% increase in the younger demographic. Cannabis use also contributed to a heightened risk, with a 37% increase overall and a 14% increase for those under 55.
Dr. Megan Ritson, a research associate at the University of Cambridge and the study’s first author, emphasized the preventable nature of these strokes. “Illicit drug use is a preventable stroke risk, but I don’t know if young people are aware how high the risk is,” she stated. This is the first comprehensive analysis to demonstrate the varying impacts of different substance use disorders on stroke incidence.
Understanding Stroke: A Disruption of Blood Flow to the Brain
Stroke occurs when blood supply to the brain is interrupted, either by a blockage (ischemic stroke) or a burst blood vessel (hemorrhagic stroke). This deprivation of oxygen and nutrients leads to the death of brain cells, resulting in a range of disabilities, including paralysis, speech impairment, and cognitive difficulties. Stroke is a leading cause of death and long-term disability worldwide, responsible for over 7 million deaths annually and leaving millions more with lasting impairments.
Dissecting the Evidence: Study Design and Limitations
Researchers pooled data from 32 studies involving over 100 million participants to assess the association between recreational drug use and stroke. The analysis revealed links between drug use and increased stroke risk, but it’s crucial to note that correlation does not equal causation. It’s possible that individuals who use drugs may have other underlying health factors that independently increase their stroke risk.
To address this potential confounding factor, the researchers conducted further analyses, examining whether genetic predispositions to drug use were also associated with a higher risk of stroke. The finding that individuals genetically predisposed to drug disorders were indeed more likely to experience a stroke strengthens the argument that the drugs themselves contribute to the increased risk, rather than simply being associated with a population already at higher risk.
How Drugs Impact the Cardiovascular System
The mechanisms by which these drugs increase stroke risk are multifaceted. Amphetamines and cocaine are known to dramatically elevate blood pressure and constrict blood vessels in the brain. This combination can lead to both hemorrhagic strokes (caused by bleeding in the brain) and ischemic strokes (caused by blockages). Cocaine also accelerates atherosclerosis – the buildup of plaque in the arteries – further narrowing blood vessels and increasing the risk of blockage. Cannabis, too, constricts blood vessels and may promote blood clot formation, contributing to stroke risk.
Beyond Stroke: The Broader Context of Rising Drug Use
The findings are particularly concerning given the global increase in recreational drug use over the past decade. According to the United Nations, approximately 300 million people worldwide consume illicit drugs regularly, with cannabis being the most widely used, followed by amphetamines and cocaine. This rising prevalence underscores the urgent need for increased awareness and preventative measures.
Specific Drug Associations: Hemorrhagic vs. Ischemic Stroke
The research also highlighted differences in how specific drugs relate to different types of stroke. Amphetamine abuse was strongly associated with hemorrhagic stroke (adjusted odds ratio of 4.95), but not with ischemic stroke. Cocaine abuse, however, was linked to both hemorrhagic (odds ratio of 2.33) and ischemic stroke (odds ratio of 2.03). Amphetamine abuse, but not cocaine, was associated with a higher risk of death following a hemorrhagic stroke (odds ratio of 2.63). This suggests that the mechanisms by which these drugs increase stroke risk may vary, influencing the type of stroke that occurs and its severity.
What the Stroke Association Advises
Juliet Bouverie, chief executive at the Stroke Association, emphasized the strain these substances place on the cardiovascular system. “These substances put a person’s cardiovascular system under huge amounts of stress which can lead to increased blood clotting, narrowing of blood vessels and damage to the circulatory system – all of which can lead to stroke,” she explained. “Regular use of cocaine can also lead to high blood pressure, which is the cause of around half of all strokes.”
The Stroke Association recommends a healthy lifestyle to reduce stroke risk, including a balanced diet, moderate alcohol consumption, regular exercise, and avoiding smoking, vaping, and illicit drug use.
Ongoing Surveillance and Future Research
Public health agencies continue to monitor trends in drug use and stroke incidence to inform prevention strategies and refine clinical guidelines. Further research is needed to fully elucidate the complex relationship between drug use and stroke, including identifying individuals most vulnerable to these risks and developing targeted interventions. Ongoing studies are also investigating the long-term effects of drug use on brain health and the potential for rehabilitation strategies to mitigate stroke-related disabilities. The findings from this review will likely prompt a re-evaluation of public health messaging regarding the risks associated with recreational drug use, particularly among young adults.
