Importantly, the effectiveness of dupilumab appears consistent regardless of how long a child has lived with asthma – a finding that broadens the potential population who could benefit from this treatment.

Asthma remains a significant health concern globally, and even with standard care, many children continue to experience troublesome symptoms and frequent exacerbations. Dupilumab offers a targeted approach, addressing the underlying type 2 inflammation that drives asthma in many patients. This latest review, published in Pediatric Pulmonology in November 2025, synthesizes emerging evidence from phase 3 clinical trials to provide a comprehensive overview of dupilumab’s efficacy and safety profile in pediatric asthma. The study was conducted by researchers from several Saudi Arabian universities and health institutions, including Qassim University and King Saud bin Abdulaziz University for Health Sciences.

Understanding Dupilumab and Type 2 Inflammation

Dupilumab is a human monoclonal antibody that specifically blocks the signaling of interleukin-4 and interleukin-13 (IL-4 and IL-13). These are key cytokines – signaling molecules that play a crucial role in type 2 inflammation. In asthma, type 2 inflammation contributes to airway hyperresponsiveness, mucus production, and eosinophil infiltration, all of which contribute to asthma symptoms. Not all asthma is type 2 driven; identifying patients with elevated levels of type 2 biomarkers, such as blood eosinophil counts (≥150 cells/µL) or fractional exhaled nitric oxide (≥20 ppb), is crucial for determining who might respond to dupilumab.

The systematic review examined data from multiple studies, including the phase 3 VOYAGE trial (NCT02948959) and its extension study, EXCURSION (NCT03560466). These trials focused on children aged 6-11 years with moderate-to-severe type 2 asthma. The analysis consistently demonstrated that dupilumab significantly reduced asthma exacerbations requiring systemic corticosteroids – often referred to as “rescue courses” – and improved lung function in these children. The review also assessed the safety of dupilumab in children. The most common adverse events reported in clinical trials were injection site reactions, conjunctivitis (pink eye), and upper respiratory tract infections. These events were generally mild to moderate in severity and rarely led to treatment discontinuation. However, as with any medication, potential risks and benefits should be carefully discussed with a qualified healthcare professional.
