However, the new research, published in the medical journal BMJ Medicine, suggests these benefits aren’t permanent. The study followed over 333,000 American veterans with type 2 diabetes for three years, comparing those who consistently took a GLP-1 medication to those who used a sulfonylurea – another common diabetes drug. Researchers found that individuals who remained on GLP-1s experienced an 18% reduction in cardiovascular risk compared to those on sulfonylureas. But this protection began to erode quickly when GLP-1s were paused.

Stopping GLP-1 therapy for as little as six months led to a 4% increase in cardiovascular risk compared to continued sulfonylurea use. After two years off the medication, the risk rose to a substantial 22% increase. “Even brief periods of discontinuations or interruptions might progressively erode and could ultimately reverse this protection, increasing the risk of cardiovascular events,” the researchers wrote.

A ‘Metabolic Whiplash’ and the Challenge of Rebuilding Protection

Study author Dr. Ziyad Al-Aly, a clinical epidemiologist at WashU Medicine and the VA Saint Louis Health Care System, describes the process of stopping GLP-1s as creating a “metabolic reversal.” Dr. Al-Aly explains that the body adapts to the improved metabolic state achieved while on the medication – lower insulin resistance, better blood pressure control, and improved cholesterol levels. When the medication is stopped, the body experiences a kind of “whiplash,” as these benefits quickly unravel, placing stress on the cardiovascular system.

This reversal isn’t solely linked to weight regain, although that is a significant factor. Cholesterol levels and markers of inflammation can spike when GLP-1s are discontinued, further exacerbating the risk. Interestingly, the study also found that simply restarting a GLP-1 after a pause didn’t fully restore the level of heart protection seen in those who consistently adhered to treatment. Dr. Al-Aly suggests that building cardiovascular protection takes time – roughly a year – while undoing it can happen in as little as six months.

A major reason people discontinue GLP-1 medications is cost. The high price tag – often around $400 per month – and limited insurance coverage can make consistent use unsustainable for many. This creates a hard dilemma, as the research clearly demonstrates the importance of uninterrupted treatment for maintaining cardiovascular benefits. The findings have implications for both patients and healthcare providers. It’s crucial for individuals considering GLP-1s to have a frank discussion with their doctor about their ability to maintain long-term access to the medication.
