HIV Status Linked to Higher TJA Complication Risk

Latest research presented at the American Academy of Orthopaedic Surgeons (AAOS) Annual Meeting suggests a potential link between HIV status and increased risk of complications following total joint arthroplasty (TJA)—hip, knee, and shoulder replacements. The study, led by Zbigniew Gugala, MD, PhD, of The University of Texas Medical Branch in Galveston, Texas, retrospectively analyzed data from the TriNetX database. Researchers compared outcomes for 2,588 patients with a preoperative HIV diagnosis to a much larger cohort of 588,903 patients without HIV. After employing propensity score matching to create comparable groups of 2,566 patients each, the analysis focused on several key postoperative outcomes.

The results indicated that individuals with HIV experienced a higher incidence of postoperative anemia, periprosthetic joint infection (infection around the implant), venous thromboembolic events (blood clots), and periprosthetic fractures. Superficial infections, wound complications, joint instability, heterotopic ossification (abnormal bone formation), and pain did not show statistically significant differences between the two groups. What is Total Joint Arthroplasty?

Total joint arthroplasty, commonly known as joint replacement surgery, is a procedure to replace a damaged joint with an artificial one. “There is literature demonstrating the negative effects of HIV on bone health as of the viral effects, immune activation and altered bone remodeling are contributing factors. But also, the effect of the new antiretroviral therapy alone might have added a compounding effect on the bone health and contribute to the postoperative complications,” Dr. Gugala stated.

These findings do not mean that individuals with HIV should avoid TJA if it’s medically necessary. Rather, they underscore the importance of a comprehensive and proactive approach to perioperative care—the period before, during, and after surgery. This includes optimizing hematologic status (blood health), enhancing measures to prevent blood clots, and implementing robust infection control strategies.

The study highlights the need for clinicians to be aware of these potential risks and to tailor treatment plans accordingly. This may involve closer monitoring for complications, adjustments to medication regimens, and consideration of bone health interventions. It’s crucial for patients living with HIV to openly discuss their condition and any medications they are taking with their orthopedic surgeon and primary care physician.

Limitations and Future Research

It’s important to acknowledge the limitations of this study. As a retrospective analysis of existing data, it cannot definitively prove a causal relationship between HIV status and postoperative complications. Other factors, such as socioeconomic status, access to healthcare, and co-existing medical conditions, could also play a role. The study also relied on data from a large database, which may contain inaccuracies or inconsistencies.

Dr. Gugala and his team are planning further research to investigate the specific effects of different antiretroviral therapies on postoperative outcomes. They aim to determine whether certain ART regimens are associated with a higher risk of complications and to identify strategies for mitigating these risks.
