IDD & Mental Health: Higher Anxiety, Depression & Care Barriers Found

Adults with intellectual and developmental disabilities (IDD) experience significantly higher rates of anxiety and depression compared to those without these conditions, and often face greater obstacles to accessing needed mental healthcare, according to a study published in JAMA Network Open on March 12, 2026. The research highlights a critical gap in mental health support for a vulnerable population and underscores the need for more equitable access to services and policies tailored to their unique needs.

Disparities in Mental Health and Access to Care

The study, led by Dimitri A. Christakis, MD, MPH, of the University of Washington, analyzed data from nearly 80,000 adults, comparing those with IDD to a control group without these disabilities. Researchers found that over 57% of adults with IDD had been diagnosed with anxiety, compared to just over 10% of those without IDD – a nearly five-fold increase. Similarly, depression diagnoses were reported in 57.1% of the IDD group versus 9.9% in the control group, representing an almost six-fold difference. These disparities extended to the severity of symptoms, with individuals with IDD reporting significantly more frequent and intense experiences of both anxiety and depression.

Beyond diagnosis rates, the study revealed substantial barriers to care for individuals with IDD. Nearly 18% reported delaying or forgoing therapy due to cost concerns, compared to just 3.5% of those without IDD. Approximately 9.2% of the IDD group did not receive any therapy or counseling at all due to financial constraints, compared to 3.2% of the control group. These financial barriers are particularly concerning given that individuals with IDD are more likely to live in poverty and rely on public insurance, as the study data showed – with nearly 20% of the IDD group having incomes below the federal poverty level and over 52% relying on public insurance.

Understanding the Scope of IDD

The study’s IDD group encompassed a range of conditions, including autism spectrum disorder and Down syndrome, as well as other genetic and developmental disabilities that affect cognitive function, communication, and self-care. Christakis emphasized that people with IDD often experience preventable and treatable health conditions that go unaddressed, contributing to a significantly shorter average lifespan – approximately 16 to 20 years less than the general population. Previous research has also linked IDD to longer inpatient psychiatric stays, highlighting the complexity of mental healthcare needs within this population.

Study Methodology and Limitations

The research utilized data from the National Health Interview Survey (NHIS) spanning 2021 to 2023, representing approximately 134 million U.S. Adults. The cross-sectional design of the study provides a snapshot of the prevalence of mental health conditions and barriers to care at a specific point in time, but it cannot establish cause-and-effect relationships. For example, while the study demonstrates a strong association between IDD and anxiety/depression, it does not prove that IDD directly causes these conditions. Other factors, such as social determinants of health and experiences of discrimination, may also play a significant role. The study relied on self-reported data, which may be subject to recall bias or social desirability bias.

Implications for Clinical Practice and Policy

The findings underscore the urgent need for increased awareness and improved access to mental healthcare for individuals with IDD. Christakis suggests that clinicians should receive specialized training in assessing and treating mental health conditions in this population, and that validated screening tools should be incorporated into routine practice. He also highlights the need for equitable reimbursement policies that adequately compensate clinicians for the additional time and expertise required to provide care to patients with IDD. Research suggests that even interventions like blood pressure medication may reduce anxiety symptoms in individuals with autism, demonstrating the potential for innovative treatment approaches.

Addressing systemic barriers to care is also crucial. This includes expanding access to affordable mental healthcare services, increasing the availability of culturally competent providers, and advocating for policies that promote inclusion and reduce stigma. The researchers emphasize the importance of including individuals with IDD in mental health policy development and research, ensuring that their voices and perspectives are represented.

What Comes Next: Addressing the Mental Health Crisis

Christakis and his colleagues are now focusing on identifying specific policy changes that can improve care for individuals with IDD through the Special Olympics Rosemary Collaboratory. This includes assessing existing state policies and developing recommendations for reforms. The goal is to create a more equitable and accessible mental healthcare system that meets the unique needs of this often-overlooked population. Further research is needed to explore the effectiveness of different interventions and to identify best practices for promoting mental well-being among individuals with IDD.
