Man Discovered to Have Rare Third Kidney During Infection Treatment

A man in Wardha, India, sought medical attention for lower back pain, a fever exceeding 102.2°F (39°C), and a burning sensation during urination. What began as a seemingly straightforward case of infection led to the unexpected discovery of a third kidney – a rare congenital anomaly – alongside evidence of kidney stones and infection. The case, detailed in a report published in Cureus, highlights the complexities of diagnosis and the importance of comprehensive imaging even when initial symptoms suggest a common ailment.

The 31-year-vintage patient presented with symptoms typical of a urinary tract infection (UTI). Initial examination revealed pain upon palpation of the right abdomen, cloudy urine, and elevated levels of leukocytes – white blood cells indicating an inflammatory response. A urine culture confirmed the presence of Klebsiella pneumoniae, a bacterium frequently associated with UTIs. Klebsiella pneumoniae infections can range from mild UTIs to more serious conditions like pneumonia.

Unexpected Findings: A Supernumerary Kidney

However, further investigation with ultrasound and CT scans revealed a surprising finding: the patient possessed a third kidney, fused to his right kidney, creating a horseshoe shape. This condition, known as a supernumerary kidney, is a rare congenital anomaly occurring due to abnormal cell division during fetal development. Fewer than 100 new cases are reported globally each year. The report notes that this particular presentation – a fused “horseshoe” kidney – is even rarer, with only five previously documented cases in medical literature.

While often asymptomatic, supernumerary kidneys can sometimes predispose individuals to kidney infections. In this case, both the patient’s left and right kidneys were swollen and contained mineral crystal accumulations, known as calculi or kidney stones. The patient’s left kidney exhibited more severe signs of infection, with calculus obstructing the left ureter – the tube connecting the kidney to the bladder. To facilitate urine drainage, doctors inserted a stent into the left ureter. He was then administered intravenous antibiotics: piperacillin and tazobactam three times daily, and levofloxacin every other day.

Following 48 hours of antibiotic treatment and hydration, the patient’s condition improved significantly. He was discharged with instructions to return for stent removal in one month and to continue monitoring and managing the kidney stones.
