New Therapies Show Promise for Immunotherapy-Resistant Lung Cancer | Science Translational Medicine

Researchers at The Ohio State University Comprehensive Cancer Center–Arthur G. James Cancer Hospital and Richard J. Solove Research Institute have identified potential new therapeutic avenues for patients battling advanced non-small cell lung cancer (NSCLC) that doesn’t respond to standard immunotherapy treatments. The findings, published in Science Translational Medicine, offer a glimmer of hope for individuals diagnosed with squamous and adenocarcinoma subtypes of NSCLC, representing a significant portion of lung cancer cases.

Understanding Non-Small Cell Lung Cancer and Immunotherapy Resistance

Lung cancer remains the leading cause of cancer death worldwide, with NSCLC accounting for approximately 80-85% of all lung cancer diagnoses. The American Cancer Society estimates that in 2024, over 234,000 Americans will be diagnosed with lung cancer, and more than 125,000 will die from the disease. Immunotherapy, which harnesses the body’s own immune system to fight cancer, has revolutionized treatment for many NSCLC patients. However, a substantial number – often more than half – do not respond to these therapies, or develop resistance over time. This study focuses on understanding why this resistance occurs and identifying potential ways to overcome it.

NSCLC is broadly categorized into several subtypes, with adenocarcinoma and squamous cell carcinoma being the most common. Adenocarcinoma tends to occur in the outer parts of the lungs and is more common in non-smokers, while squamous cell carcinoma is often linked to a history of smoking. The specific genetic and molecular characteristics of each subtype can influence treatment response.

The Study’s Approach and Key Findings

The research team, led by scientists at the OSUCCC–James, investigated the underlying mechanisms of immunotherapy resistance in NSCLC. Their work centered on analyzing tumor samples from patients who had not responded to immunotherapy. The study didn’t disclose the exact sample size, but focused on identifying specific biomarkers – measurable indicators of a biological state – that correlated with resistance.

The researchers discovered that a particular signaling pathway, involving a protein called DDR1, was abnormally activated in the tumors of non-responding patients. DDR1 is a receptor tyrosine kinase, meaning it’s a protein that sits on the cell surface and transmits signals into the cell, influencing growth and survival. The study suggests that increased DDR1 activity effectively shields cancer cells from immune attack, contributing to immunotherapy resistance.

Importantly, the team found that inhibiting DDR1 in laboratory models – both in cell cultures and in animal studies – restored sensitivity to immunotherapy. This suggests that targeting DDR1 could potentially re-sensitize tumors to immune-based therapies.

What Does This Signify for Patients?

These findings are preliminary, but they offer a promising new direction for research and potential treatment development. It’s crucial to understand that this study does not represent an immediate change in clinical practice. Further research is needed to confirm these findings in larger patient cohorts and to develop safe and effective DDR1 inhibitors for use in humans.

The identification of DDR1 as a potential therapeutic target is significant as it provides a specific, actionable pathway to explore. Instead of a broad-spectrum approach, researchers can now focus on developing drugs that specifically block DDR1 activity, potentially minimizing side effects and maximizing efficacy.

Limitations and Future Directions
