Recent findings suggest that implementing an operations room (OR) efficiency and analytics program can lead to significant improvements in surgical throughput, particularly for total joint arthroplasty procedures. This approach, centered on detailed time analysis, offers a pathway to optimize workflows and potentially reduce wait times for patients needing hip and knee replacements.

Radical Time Transparency Drives Surgical Improvements

George N. Guild III, MD, of Total Joint Specialists in Atlanta, presented data at the American Academy of Orthopaedic Surgeons Annual Meeting highlighting the benefits of such a program. Dr. Guild emphasized the importance of “radical time transparency,” extending beyond the OR itself to encompass turnover times – the period between surgeries when the room is prepared for the next patient. “It has changed the way I do perioperative management, and it is an invaluable part of our surgery center,” he stated. The program utilized a cloud-based OR efficiency and analytics platform (1Team Surgical, Zimmer Biomet) to timestamp surgical workflow data for 106 total hip arthroplasties (THA) and 214 total knee arthroplasties (TKA).

The analysis was structured in three phases: a diagnostic phase to establish baseline times, a working phase where efficiency improvements were implemented based on the data, and a follow-up phase to assess the impact of the changes. Researchers tracked key metrics including total OR time, incision to implant time, incision to close time, wheels-in to incision time, OR turnover time, and surgeon turnover time.

Total Knee Arthroplasty: Significant Gains in Efficiency

For TKA procedures, the study revealed notable differences between the diagnostic and follow-up phases. Although there was no significant change between the diagnostic and working phases, researchers observed a significant reduction in overall OR time when comparing the diagnostic and follow-up phases. Further analysis showed significant improvements in incision to implant time, incision to closing time, wheels-in to incision time, and surgeon turnover time during the follow-up phase. This suggests that targeted interventions based on the time data were effective in streamlining the TKA process.

Total Hip Arthroplasty: Focused Improvements in Key Areas

The results for THA procedures showed a non-significant trend toward shorter OR times. But, the study did identify statistically significant reductions in wheels-in to incision time and surgeon turnover time in the follow-up phase compared to the diagnostic phase. This indicates that the program was particularly successful in optimizing the preparation and transition phases of THA procedures.

Dr. Guild explained to Healio that the ability to meticulously track time allows for targeted improvements in surgical flow and perioperative management. “When you have those time stamps, what gets measured gets improved,” he said. This data-driven approach enables surgical teams to identify bottlenecks and implement strategies to enhance efficiency.
