OSA Costs: Workplace Productivity Loss Exceeds CPAP Treatment Costs

The economic impact of obstructive sleep apnea (OSA) extends beyond the cost of treatment like CPAP therapy, with new research indicating that workplace productivity losses associated with the condition actually exceed those costs. A study published in Thorax reveals significant productivity declines linked to OSA in both the United States and the United Kingdom, reinforcing the importance of early diagnosis and intervention.

Prevalence and Economic Burden

Researchers analyzed data from over 4,300 adults in the U.S. And U.K., utilizing self-reported symptoms – specifically, reported breathing pauses during sleep combined with excessive daytime sleepiness occurring at least three nights a week – to identify individuals likely experiencing OSA syndrome. The study, led by Matt Lechner, MD, PhD, FRCS, FHEA, professor at University College London and consultant ENT surgeon, aimed to quantify the economic consequences of this often-underdiagnosed condition.

The findings demonstrate a substantial economic toll. In the U.S., the estimated total annual productivity loss due to OSA reached $180.2 billion. The U.K. Experienced an estimated loss of £4.22 billion. On an individual level, the annual productivity loss for a worker with OSA was $3,727.40 in the U.S. And £1,840 in the U.K. These figures surpass the annual cost of continuous positive airway pressure (CPAP) treatment, which averages $1,660.97 per patient in the U.S. And £1,363 in the U.K.

“These findings reinforce that symptoms associated with OSA are common and should not be dismissed as benign,” Dr. Lechner told Healio. “Clinicians play a crucial role in recognizing early warning signs, arranging appropriate investigations and referrals, and supporting timely treatment initiation.”

Understanding OSA and its Impact

Obstructive sleep apnea is a condition where the upper airway repeatedly collapses during sleep, leading to pauses in breathing. These pauses disrupt sleep quality and can result in daytime sleepiness, impaired cognitive function, and increased risk of cardiovascular disease and other health problems. Fatigue and reduced work productivity are commonly reported by individuals with OSA, impacting their ability to perform tasks effectively and safely.

The study’s methodology involved a computable general equilibrium macroeconomic model, utilizing the prevalence of OSA syndrome within the working populations of both countries. This model allowed researchers to estimate the broader economic impact of the condition. The research fellow, Umar Rehman, BMBS, emphasized the difficulty in accurately assessing OSA prevalence due to many individuals remaining undiagnosed or presenting with associated illnesses that mask the underlying sleep disorder.

Limitations and Future Research

While the study provides compelling evidence of the economic burden of OSA, it’s important to acknowledge its limitations. The reliance on self-reported symptoms to define OSA syndrome introduces the possibility of misdiagnosis. Individuals may experience symptoms similar to OSA without actually having the condition, as confirmed by a sleep study. The study did not account for potential confounding factors such as obesity, smoking, or other co-existing medical conditions that could contribute to both sleep symptoms and reduced productivity.

As Indira Gurubhagavatula, MD, MPH, spokesperson and Board Member of the American Academy of Sleep Medicine, notes in a Healio perspective, prior studies have shown that symptom-based diagnosis alone is often inaccurate. She highlights the need for objective diagnostic measures, such as polysomnography (sleep studies), to confirm OSA diagnoses and accurately link productivity losses to the condition.

Dr. Lechner and his team acknowledge these limitations and suggest future research should incorporate objective diagnostic measures and longitudinal follow-up studies. Expanding economic modeling to other countries could also provide a more comprehensive understanding of the global impact of OSA.

Implications for Public Health and Policy

The findings of this study underscore the need for increased awareness of OSA and its potential consequences. Raising public awareness can encourage individuals experiencing symptoms to seek clinical evaluation and treatment. Early diagnosis and effective management of OSA, through interventions like CPAP therapy, lifestyle modifications, or surgical options, could potentially mitigate productivity losses and reduce the overall economic burden of the condition.

The researchers suggest that prioritizing OSA in national health policies could be economically advantageous. Investing in screening programs, improving access to diagnostic testing, and promoting awareness campaigns could lead to earlier detection and treatment, ultimately benefiting both individuals and the economy.
