KABUL, Afghanistan — The fragile truce between Pakistan and Afghanistan, announced Wednesday, comes as Kabul begins the grim task of assessing the human cost of recent escalating conflict. The most devastating single event occurred Monday night, when an airstrike destroyed a significant portion of the Omid Addiction Treatment Hospital in the Afghan capital, leaving a trail of loss and unanswered questions.

Initial reports from Taliban officials indicate the hospital was struck by a Pakistani airstrike, resulting in over 400 deaths and more than 250 injuries. However, these figures are significantly higher than those provided by the United Nations Assistance Mission in Afghanistan (UNAMA), which currently estimates at least 143 fatalities and 119 wounded. The incident has sparked international condemnation and fueled tensions between the two neighboring countries.

The Omid Addiction Treatment Hospital, or the Support and Treatment Center, Omid, was a crucial facility, treating approximately 3,000 individuals struggling with substance use disorders. The scale of the destruction is immense, with large sections of the rehabilitation compound completely leveled. Rescuers are still sifting through the rubble, and families are desperately seeking information about their loved ones. Abdul Basir Watan, searching for his cousin Zamarek who was undergoing treatment at the hospital, described a scene of anguish and uncertainty, with fears mounting that many victims may remain unidentified. Images circulating on social media demonstrate the extent of the devastation and the desperate search for survivors.

Conflicting Accounts and Ongoing Investigation

Pakistan maintains that its air force only targeted “military and terrorist infrastructure,” denying any intentional strike on the hospital. Pakistani officials claim the strikes were aimed at disrupting militant groups operating near the border. However, Georgette Gagnon, officer-in-charge of UNAMA, has stated the facility was “a well-known rehabilitation center” operated by the Taliban’s interior ministry, contradicting Pakistan’s assertions. UNAMA’s investigation is ongoing, aiming to clarify the circumstances surrounding the attack and determine the extent of civilian casualties.

The attack occurred amidst a period of heightened tensions between Afghanistan and Pakistan. Islamabad accuses the Taliban regime of providing safe haven to Islamist groups, including the Tehreek-e-Taliban Pakistan (TTP) and the Balochistan Liberation Army (BLA), which have been responsible for attacks within Pakistan. In response, Pakistan has implemented border closures, halted trade, and expelled millions of Afghans over the past year. These actions have further strained relations and contributed to the recent escalation of violence.

A History of Cross-Border Conflict

This latest incident builds upon a history of cross-border strikes and accusations. Similar tensions peaked last October, leading to a fragile ceasefire mediated by Qatar and Turkey. However, negotiations subsequently broke down, paving the way for the recent surge in hostilities. The current five-day ceasefire, announced in conjunction with the Muslim holiday of Eid, offers a temporary respite, but the underlying issues remain unresolved.

Ibrahim Bahiss, an Afghan expert with the International Crisis Group, suggests that Pakistan’s approach is complicated by its tendency to link various internal conflicts to external actors. “They’ve lumped everything together,” Bahiss explains. “The TTP is a Taliban proxy. The BLA is an Indian proxy. And then the Taliban are Indian proxies.” He argues that this broad categorization obscures a more nuanced understanding of the situation and hinders effective conflict resolution. Bahiss’s analysis highlights the complexities of regional dynamics and the challenges in addressing the root causes of instability.

The Human Cost and the Search for Answers

Beyond the political maneuvering and strategic calculations, the human cost of the conflict is devastatingly clear. Families in Kabul are left to grieve their loved ones and search for answers. Sahil, searching for his brother Mohammad Yahya, faced the agonizing reality of identifying bodies at the Emergency Hospital, ultimately unable to find his brother among the charred remains. The stories of loss and desperation underscore the urgent demand for a lasting resolution to the conflict.

The United Nations has expressed concern over the targeting of the Omid addiction treatment center and called for an immediate halt to violence. UN officials are emphasizing the importance of protecting civilians and upholding international humanitarian law.

As the ceasefire takes hold, the focus shifts to the long-term prospects for peace and stability in the region. The coming days will be critical in determining whether this temporary truce can evolve into a more sustainable solution, or whether the cycle of violence will resume. The families of those lost in the hospital strike, and countless others affected by the conflict, deserve nothing less than a lasting peace.
