  • Medications: Injections or oral medications used to treat erectile dysfunction (ED), particularly when misused or taken in excess.
  • Other Drugs: The use or misuse of medications like antidepressants, anti-anxiety drugs, antipsychotics, and blood thinners. Illicit drug use, including marijuana, ecstasy, and cocaine, can also be a factor.
  • Underlying Medical Conditions: Conditions affecting blood cell flexibility, such as sickle-cell anemia, or those increasing the risk of blood clots, like diabetes.
  • Trauma: Injury to the pelvic or genital area, potentially rupturing an artery and disrupting normal circulation.
  • Other Rare Causes: Spinal cord injury, venomous spider bites (like black widows), carbon monoxide poisoning, certain cancers, and even the use of “aphrodisiacs” like Spanish fly.

Recognizing the Emergency and Seeking Help

While uncommon, priapism is considered a medical emergency. Individuals should seek immediate medical care if they experience excessive swelling, pain, or an unusually prolonged erection. Repeated, unwanted erections lasting 2-3 hours, known as stuttering priapism, also warrant a visit to a physician. Attempting self-treatment is strongly discouraged.

Treatment Options for Priapism

Treatment for priapism depends on the underlying cause and severity of the condition. Medical interventions may include:

  • Medications: To decrease blood flow to the penis.
  • Ice Packs: To reduce swelling.
  • Aspiration: Draining excess blood from the penis using a needle, often followed by a saline flush.
  • Surgical Intervention: Repairing a ruptured artery or creating a shunt to restore normal blood flow.
  • Intracavernous Injection: Injecting medication to narrow the arteries and reduce swelling.

Prognosis and Prevention

With prompt treatment, the prognosis for priapism is generally positive. Preventing recurrence involves identifying and avoiding triggers, hormone therapy, medication adjustments, or managing underlying medical conditions. Priapism can affect males of any age, though it’s most common in boys aged 5-10 and men aged 20-50.
