A sense of cautious optimism is growing among dermatologists regarding the treatment of pyoderma gangrenosum, a rare and challenging chronic wound disease. Although few medications currently offer reliable relief, emerging therapies and a deeper understanding of the condition’s underlying mechanisms are fueling hope for improved outcomes. This positive outlook comes as researchers continue to investigate new treatment avenues and refine diagnostic approaches for this often-misunderstood illness.
Understanding Pyoderma Gangrenosum
Pyoderma gangrenosum affects roughly 1 in every 100,000 Americans, manifesting as small blisters that rapidly coalesce into large, painful open sores. Pain levels can be a key indicator of treatment response, but diagnosis remains complex. Because the condition is often a “diagnosis of exclusion,” meaning other potential causes must be ruled out first, a skin biopsy is crucial. As Robert S. Kirsner, MD, PhD, chairman and professor at the department of dermatology and cutaneous surgery at University of Miami Miller School of Medicine, emphasizes, even when a case appears textbook, a biopsy can reveal unexpected findings. “I remember seeing a patient and thinking What we have is classic pyoderma gangrenosum,” Kirsner said. “Then, that little person on my shoulder said to do a biopsy, so I did, and it came out to be squamous cell carcinoma.”
Current Treatment Options and Their Limitations
Currently, dermatologists primarily rely on cyclosporine or prednisone to manage pyoderma gangrenosum. The choice between the two often hinges on the patient’s overall health profile. For individuals with diabetes, cyclosporine may be preferred, while those with hypertension might benefit more from prednisone. Tumor necrosis factor (TNF) inhibitors, particularly infliximab, have likewise demonstrated effectiveness, becoming a mainstay in treatment protocols based on available data. However, these options aren’t without their drawbacks, and the search for more targeted and effective therapies continues.
The Promise of Spesolimab and Beyond
The horizon for pyoderma gangrenosum treatment is brightening with the development of novel therapies. Spesolimab (Spevigo, Boehringer Ingelheim), an interleukin-36 inhibitor initially approved for generalized pustular psoriasis, is showing promise. Its mechanism of action and early case report successes suggest it could be a valuable addition to the treatment arsenal. Phase 3 trials for vilobelimab are also underway, further expanding the potential for new treatment options.
Kirsner notes a broader trend: even if a drug isn’t initially developed specifically for pyoderma gangrenosum, it may become a viable treatment option as researchers uncover its potential benefits. “It is an exciting time where optimism is high,” he stated. This phenomenon is similar to what has been observed with hidradenitis suppurativa, where treatments originally intended for other conditions have proven effective.
The Importance of Accurate Diagnosis and a Multi-faceted Approach
Beyond new medications, refining diagnostic accuracy remains paramount. The “diagnosis of exclusion” nature of pyoderma gangrenosum underscores the need for thorough investigation to rule out other conditions that can mimic its symptoms. A biopsy is not merely a confirmatory step, but a critical safeguard against misdiagnosis and inappropriate treatment.
Effective management also requires a holistic approach, considering the patient’s overall health and tailoring treatment to their specific needs. The choice between cyclosporine and prednisone, for example, highlights the importance of individualizing therapy based on co-existing conditions.

The field is actively pursuing several avenues of research. Ongoing clinical trials are evaluating the efficacy of new therapies like vilobelimab, while researchers continue to investigate the underlying immunological mechanisms driving pyoderma gangrenosum. The U.S. Embassy in Amman, Jordan, recently issued a security alert, highlighting the importance of staying informed about global health and safety concerns, a principle that extends to staying abreast of developments in complex medical conditions like pyoderma gangrenosum.
