Once reserved for the most severe cases of sleep apnoea, CPAP use is now expanding as awareness of sleep disorders grows and diagnostic technologies improve. But the story of Nick, and the increasing prevalence of CPAP, reveals a complex interplay between social impact, medical understanding, and technological advancement in addressing a condition that affects millions.

From Social Problem to Medical Diagnosis

Nick’s experience wasn’t unusual. For years, he’d managed his snoring with humor, but underlying symptoms – frequent daytime tiredness, even experiencing micro-sleeps during meetings and on public transport – began to raise concerns. Humans have long sought ways to combat snoring. Solutions throughout history have been remarkably inventive – and sometimes, rather drastic. An Egyptian medical papyrus from around 1550 BCE recommended thyme as a remedy, while soldiers during the American War of Independence were reportedly sewn into uniforms with cannonballs to prevent them from sleeping on their backs. These historical attempts underscore the enduring social impact of snoring, even before its medical implications were fully understood.

The modern medical approach to sleep apnoea began to take shape in the 20th century. Early treatment involved invasive procedures like tracheostomy, creating a surgical opening in the trachea to bypass airway obstructions. However, the development of the CPAP machine by Professor Colin Sullivan in the 1980s revolutionized treatment. The University of Sydney details the history of this breakthrough, which has since been commercialized by ResMed, a company whose value has quadrupled in the last decade.

Expanding Access and Rising Demand

Initially, CPAP machines were primarily used for severe cases of sleep apnoea. However, there’s been a noticeable increase in prescriptions for milder cases as doctors become more aware of the condition and its subtle symptoms. This shift is also fueled by the growing availability of consumer sleep-tracking technology. Professor Peter Cistulli, a sleep medicine expert at the University of Sydney, notes the emergence of “orthosomnia” – an obsessive preoccupation with achieving a perfect night’s sleep – driven by data from wearable devices. The Guardian explored this phenomenon in detail, highlighting the potential downsides of hyper-focusing on sleep metrics.

While CPAP remains a highly effective treatment, it’s not without its challenges. Some individuals find the machine uncomfortable or claustrophobic, likening it to “a vacuum cleaner turned the other way.” Dr. Donald Lee, a respiratory and sleep physician, emphasizes that motivation to use the machine is key, and that it must demonstrably improve a person’s well-being. The traditional image of a sleep apnoea patient – an overweight, middle-aged man who drinks alcohol – is becoming increasingly outdated. Cistulli notes that they are now seeing more subtle forms of the condition in younger individuals and across genders. Schachter adds that women have historically been under-treated for sleep apnoea, often experiencing significant symptoms of tiredness even with milder breathing disturbances. There’s growing recognition of “upper airways resistance syndrome,” where breathing difficulties cause sleep disruption without meeting the full criteria for sleep apnoea.

What to Do If You Suspect a Problem

If you or someone you know experiences persistent loud snoring, daytime sleepiness, or other symptoms suggestive of sleep apnoea, it’s important to consult a healthcare professional. While snoring isn’t always indicative of a medical problem, it warrants investigation. Cistulli emphasizes that treating snoring proactively may even prevent the development of sleep apnoea. Beyond health concerns, addressing snoring can significantly improve relationships, as disturbed sleep impacts both the snorer and their partner.

Treatment options extend beyond CPAP machines. Mouth splints, nasal dilators, and weight-loss programs can be effective for mild to moderate cases. It’s crucial to discuss these options with a doctor to determine the most appropriate course of action. Lee cautions against purchasing CPAP machines from third-party retailers without medical guidance, as proper setup and pressure calibration are essential for safe and effective use.

as Nick discovered, addressing sleep apnoea can be transformative. He compares receiving a CPAP machine to getting glasses after years of gradual vision loss. “It’s just not that sizeable of a deal,” he says. “In a world of trade-offs, it’s absolutely no contest. Self-care is sexy.”
