A Shift in Function During the Trump Administration

Despite the core mission of the USPSTF – evaluating evidence and providing recommendations on preventive services – remaining unchanged, Dr. Krist emphasizes a significant disruption in its functionality. “The problem is that they’ve not been allowed to meet and they’ve not been allowed to carry out their function,” he explained. Meetings scheduled for July and November of 2025 were canceled by HHS Secretary Robert F. Kennedy Jr., and the release of finalized recommendations, including those on cervical cancer screening and perinatal depression screening, have been stalled despite being completed months ago. This cancellation of meetings raised concerns among medical groups last year.

The Ripple Effect on Clinical Practice

The impact of this disruption extends far beyond bureaucratic delays. Dr. Krist highlights the daily reliance clinicians place on USPSTF recommendations. “I apply the task force recommendations 100 times a day, and that’s not figurative,” he stated. Without updated guidance, primary care physicians are left without a clear, evidence-based framework for preventive care, potentially leading to inconsistent practices and missed opportunities to improve patient health. This also has economic implications, as USPSTF recommendations are linked to coverage mandates under the Affordable Care Act, ensuring access to vital preventive services.

The USPSTF’s recommendations aren’t simply about what *to* do, but also what *not* to do. By identifying services with limited evidence or potential harm, the task force helps payers avoid wasteful spending and ensures resources are allocated effectively. A paralyzed USPSTF undermines this crucial function.

A Shrinking Task Force and the Threat of Overhaul

Compounding the issue is the natural attrition of task force members. As terms expire, members rotate off, but replacements are not being appointed. With five members rotating off at the end of 2025, and a stalled nomination process, the task force is dwindling in size – from 16 to 11 members, and projected to fall to eight by the end of 2026. This raises the specter of a complete overhaul, potentially mirroring the changes seen at the CDC’s Advisory Committee on Immunization Practices (ACIP). Former task force members have warned that such a fate could have adverse implications.

Dr. Krist expressed deep concern about this possibility, stating that a reconstituted task force lacking the trust of the primary care community would be detrimental to public health. “If Kennedy does what he did to ACIP to the task force, primary care in the country is not going to listen to the task force anymore, and we’re going to lose that North Star for prevention.”

The Importance of Evidence-Based Practice Centers

Beyond the task force members themselves, Dr. Krist also emphasized the importance of the Evidence-Based Practice Centers (EPCs) that conduct the systematic reviews underpinning USPSTF recommendations. These centers provide the rigorous, unbiased evidence that informs the task force’s decisions. If funding for EPCs is curtailed, the expertise and infrastructure necessary for evidence-based guideline development could be lost.

Dr. Krist’s assessment is stark, but he believes a solution is straightforward: “Kennedy just needs to get out of the way and let the task force do the work that it’s so great at doing.” He also urges clinicians to advocate for the USPSTF, voicing their reliance on its recommendations and emphasizing its value to patient care.
